Terms & Conditions

These terms and conditions are maintained by Ashley Millington-Hogg, Registered Massage Therapist, for clients booking treatment at my practice in Brantford, Ontario. 1. Professional scope

I am a Registered Massage Therapist in Ontario, governed by the College of Massage Therapists of Ontario (CMTO). Services provided include therapeutic massage, treatment of temporomandibular joint dysfunction (TMJ), cupping massage, and neurofunctional electroacupuncture. These services are intended to support muscular, nervous-system, and stress-related concerns and do not replace medical diagnosis or treatment by a physician or other regulated healthcare provider.

2. Booking and appointments

Appointments are by booking only and can be scheduled through my online booking system, Jane App. Same-day and walk-in appointments are not generally available. When you book, please provide accurate contact and health information so I can plan a safe, appropriate treatment.

3. Cancellation and missed appointments

If you need to cancel or reschedule, please provide at least 24 hours notice. This allows me to offer the time to another client. Late cancellations or missed appointments may be subject to a fee. Repeated missed appointments may result in pre-payment being required for future bookings.

4. Late arrivals

If you arrive late, your session may be shortened to respect the next client’s appointment time. The full scheduled fee will still apply.

5. Health history and informed consent

Before your first visit, you will complete a health-history form. It is your responsibility to disclose any medical conditions, medications, allergies, recent surgeries, or other factors that could affect treatment safety. I will explain the proposed treatment, including any risks or alternatives, and obtain your informed consent before beginning. You may withdraw consent or pause treatment at any time.

6. When treatment may not be appropriate

Massage therapy and acupuncture are not appropriate for every condition. I reserve the right to refuse or modify treatment if it may be unsafe, or to refer you to another healthcare provider when appropriate. If you are unsure whether treatment is right for you, please contact me before booking or speak with your physician.

7. Payment, HST, and direct billing

All listed prices include HST. Payment is due at the time of service unless direct billing to your insurance provider has been arranged in advance. You are responsible for confirming your eligibility, coverage details, yearly maximums, and any deductibles with your insurer. If direct billing is declined for any reason, you are responsible for full payment at the time of your appointment.

8. Receipts and insurance

Receipts are provided for all treatments and may be submitted to your extended health benefits provider. It is your responsibility to confirm that your plan covers massage therapy and/or acupuncture and recognizes my registration credentials.

9. Privacy

Your personal and health information is collected, used, and stored in accordance with my Privacy Policy. Treatment records are retained for seven years and then securely destroyed.

10. Conduct

Clients are expected to behave respectfully. Inappropriate, threatening, or harassing behaviour will result in immediate termination of the session and may lead to refusal of future appointments.

11. Limitation of liability

While every effort is made to provide safe and effective care, outcomes cannot be guaranteed. I am not liable for adverse effects resulting from undisclosed health information, failure to follow aftercare recommendations, or conditions outside the scope of massage therapy or acupuncture.

12. Changes to these terms

These terms may be updated from time to time. The current version will always be available at this page, with the updated date noted at the top.

13. Governing law and contact
